Work Environment

Key Responsibilities

• Conduct thorough assessments of clients' social, emotional, and environmental challenges to formulate effective intervention strategies.
• Develop and implement personalized service plans that include measurable goals, interventions, and progress evaluations.
• Provide individual and family counseling sessions focused on addressing life challenges, trauma, and interpersonal issues.
• Engage in crisis intervention, conducting risk assessments and establishing safety plans for clients in immediate distress.
• Collaborate with community organizations and agencies to coordinate services and act as an advocate for clients, ensuring they have access to vital resources.
• Participate in community outreach efforts aimed at raising awareness of mental health issues and the services offered by Evolve Mind Therapy Collective.
• Attending regular team meetings, case conferences, and staff training sessions to enhance collaborative practices and shared learning.

Patient Population / Client Focus

The primary focus of this position will be on addressing the needs of school-aged children and their families who face a variety of psychosocial challenges. You will play a critical role in collaborating with school staff to identify at-risk students and provide them with the necessary interventions, support, and resources to foster their overall well-being.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Demonstrated knowledge of community resources, social justice issues, and trauma-informed care principles.
• Exceptional communication skills, cultural competence, and the ability to problem-solve effectively in complex situations.

Preferred Experience

• Previous experience working in a social service agency or similar environment.
• Experience with school-based social work or familiarity with educational settings.

Compensation and Benefits
