  • Guide clients through the process of developing their personalized service plans, connecting them with appropriate community resources and services such as housing, healthcare, and employment assistance.
  • Provide crisis intervention and risk assessment services as necessary, ensuring client welfare and safety planning.
  • Collaborate with external organizations and community agencies to coordinate comprehensive services and advocate for client needs effectively.
  • Engage in outreach activities to elevate awareness of our services within the community, and promote access to available resources.
