This position will primarily be remote, providing you the flexibility to balance your personal and professional life. Although you will primarily work from home, our hospice team is a close-knit community that regularly engages through video calls and virtual meetings, ensuring ongoing support and collaboration. • Conduct comprehensive evaluations and assessments to determine the needs of patients and their families.
• Collaborate with multidisciplinary teams to develop and implement Individualized Family Service Plans (IFSPs) tailored to each patient's unique circumstances.
• Monitor patient progress, actively participating in the assessment and adjustment of care strategies.
• Provide direct counseling and support to families, empowering them with the skills necessary to navigate the complexities of end-of-life care.
• Adhere to federal and state regulations to ensure compliance and quality care delivery.
• Foster strong relationships with children, families, and healthcare professionals to enhance collaborative care efforts.
• Utilize evidence-based strategies aligned with Natural Learning Environment Practices to ensure optimal patient outcomes.
• Educate families about available resources, community programs, and effective coping strategies for developmental delays in their children.

In this role, you will primarily work with children with disabilities or developmental delays, as well as their families. Our Children's Services provide critical support to infants and toddlers in their formative years, focusing on enhancing their physical, cognitive, social, emotional, and communication skills. Your expertise will be essential in guiding these families through challenging times, offering not just emotional support, but also practical advice and resources.

• A Master's or Doctorate in Social Work is required to qualify for this role.
• Current licensure from the State Department of Public Health.
• Certification as a Licensed Clinical Social Worker (LCSW) is mandatory.
• A valid driver's license is required to access clients in various settings if needed.
• Strong organizational and time management skills are essential for managing caseloads effectively.
• Ability to communicate effectively across diverse populations, with bilingual abilities preferred.

• At least one year of experience in early intervention services or working with children with developmental challenges.
• Familiarity with Natural Learning Environment Practices and evidence-based intervention strategies.

This role offers a competitive salary ranging from USD 90,000 to 105,000 annually, commensurate with experience and qualifications. Our benefits package includes:
