Key Responsibilities
  • Maintain a manageable caseload, cultivating rapport with clients to develop meaningful therapeutic relationships.
  • Conduct regular meetings to assess client progress, addressing any emerging challenges while preparing for potential discharge when appropriate.
  • Create, implement, and monitor individualized, person-centered treatment plans tailored to the unique needs of each client.
  • Document and maintain comprehensive progress notes and treatment documentation, utilizing electronic health record systems efficiently.
  • Administer intake assessments to identify client needs and establish the groundwork for therapeutic interventions.
  • Provide individual therapy sessions via telehealth, applying evidence-based practices tailored to client preferences and goals.

  • Collaborate effectively with multidisciplinary healthcare providers to coordinate comprehensive care plans and ensure continuity of services.

Patient Population / Client Focus

Qualifications
