• Conduct thorough assessments of patients, utilizing evidence-based tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to accurately diagnose and evaluate mental health statuses.
• Develop and implement individualized care plans that respect cultural diversity and address the unique needs of the patient population, ensuring a holistic approach to treatment.
• Monitor and document patients’ progress meticulously, adapting interventions and strategies as necessary to support continued growth and recovery.
• Engage collaboratively with patients and their families to co-create comprehensive treatment plans that align with their desired outcomes and address immediate concerns.
• Coordinate with various treatment providers to facilitate seamless referrals, ensuring continuity of care and effective communication throughout the treatment network.
• Mentor and provide educational opportunities for interns, volunteers, and peers, fostering a culture of knowledge sharing and professional development within the organization.
• Participate in peer reviews, clinical collaboration, and chart audits under the direction of clinical supervisors, enhancing clinical practices and team efficacy.

Your primary focus will be health and wellness among individuals and families affected by cancer. This includes providing outpatient therapy designed to address the psychosocial aspects of living with cancer—from coping mechanisms and emotional resilience to counseling for grief and loss. You will play a vital role in helping these patients navigate their emotional landscapes and improve their overall quality of life.

• A Master’s degree in Social Work from an accredited institution.
• A current and valid State License as a Licensed Clinical Social Worker (LCSW).

• Experience working in outpatient settings with a focus on oncology or chronic illness is highly desirable.
• Familiarity with evidence-based therapeutic methodologies tailored to this patient demographic enhances your contribution to effective patient care.
• Skills in crisis intervention and familiarity with community resources that can support patients and families will further enrich our multidisciplinary approach.
