Our Community Health Clinic adopts a comprehensive approach to care, focused on providing services to children and their families, with an emphasis on the delivery of early intervention and family-centered practices. We utilize technology to facilitate ongoing communication and coordination among team members and families.

Key Responsibilities
  • Conduct initial evaluations and ongoing assessments to determine the developmental needs of children enrolled in our programs.
  • Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), working collaboratively with families to identify goals and strategies.
  • Monitor and report on developmental outcomes, ensuring that best practices are followed while adhering to all Federal and State regulations.
  • Provide emotional and educational support to families, equipping them with skills and strategies to enhance their child’s development.
  • Deliver support through the coaching model and Natural Learning Environment Practices, engaging with families in their natural settings.
  • Maintain comprehensive documentation of assessments, sessions, and outcomes using the clinic's electronic health record system.
Patient Population / Client Focus

Your work as an LCSW will directly impact young children, from infants to toddlers, who experience disabilities or developmental delays. You will focus on helping them develop essential skills, including physical, cognitive, social/emotional, and communication abilities. By engaging closely with families, you will play a crucial role in their child’s growth and development journey.

Qualifications
  • A Master's Degree in Social Work (MSW) or a Doctorate of Social Work (DSW).
  • Current LCSW licensure recognized by the Department of Public Health.
  • A valid driver’s license to access family homes as needed.
  • Proficiency in adapting therapeutic approaches to meet the unique needs and learning styles of each child.
  • Commitment to practicing within Natural Learning Environment frameworks to promote child-centered development.
  • Outstanding organizational abilities and time management skills.
  • Effective communication skills, fostering collaborative relationships with families and team members.
Preferred Experience
  • At least one year of experience working directly with children in early intervention settings.
  • Bilingual candidates are encouraged to apply to better serve our diverse population.
  • Demonstrated ability to implement evidence-based practices tailored to individual client needs.
Compensation and Benefits

We offer a competitive annual salary range of USD 75,000 – 80,000, along with a robust benefits package that includes:

  • Sign-on bonus for eligible candidates.
  • Comprehensive health, dental, and vision insurance plans.
  • 401(k) retirement plan with employer matching contributions.
  • Generous paid time off and sick leave.
• Life insurance and profit-sharing options.
  • Discounts with various vendors and flexible spending accounts.
  • Support for continued education through tuition reimbursement.
  • Flexible scheduling to promote work-life balance.
  • Incentives for employee referrals.
