Job description

Greater San Francisco Area | Field-Based Role

Make a meaningful impact—one recovery at a time

We are dedicated to improving outcomes for injured workers, and we’re seeking an experienced Field Case Manager (RN) to support patients throughout the Greater San Francisco region. This is an ideal opportunity for a clinically strong, independent RN who thrives in the field, enjoys collaboration, and is motivated by helping individuals safely return to work.

About the Role

As a Field Case Manager, you will coordinate medical care for injured workers within the workers’ compensation system. You’ll use your clinical expertise and advocacy skills to manage complex cases and influence positive return-to-work outcomes—all while building relationships with providers and employers.

Work Territory & Logistics

  • Regional Coverage: This is a field-based position covering a territory within approximately 2 hours of your home.

  • Travel: Includes travel to medical appointments, employer sites, and stakeholder meetings within your assigned local territory.

  • Mileage: Full mileage reimbursement is provided for all professional travel.

What You’ll Do

  • Independently assess injured workers and develop treatment plans.

  • Serve as a liaison between injured workers, medical providers, and employers.

  • Attend physician appointments to ensure clear communication and adherence to care plans.

  • Stay current on workers’ compensation regulations and best practices.

What We’re Looking For

  • Required: Active Registered Nurse (RN) license; prior case management experience (workers’ comp or occupational health preferred).

  • Required: Strong clinical assessment skills and ability to manage a field-based caseload independently.

  • Preferred: Certified Case Manager (CCM) certification; experience in hospital or rehabilitation settings.

Why Join Us?

  • Purpose-driven work with high autonomy in clinical decision-making.

  • Competitive mileage reimbursement and flexible, field-based scheduling.

  • A collaborative culture that values your expertise in influencing patient health outcomes.

If you are an RN Case Manager ready to trade the hospital halls for the autonomy of a field-based role, we would love to hear from you.
