In this role, you will be responsible for:

• Conducting comprehensive clinical assessments, understanding patient needs, and formulating detailed treatment plans that align with best practices for hospice care.
• Providing therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to address a variety of mental health concerns.
• Documenting clinical notes and progress in patient records while ensuring compliance with HIPAA regulations to uphold patient confidentiality and care standards.
• Working collaboratively with interdisciplinary team members to coordinate care, discussing treatment plans in case consultation meetings, and implementing any necessary adjustments.
• Educating patients and their families about mental health issues, treatment options, and available community resources to empower them during the treatment process.
• Staying informed on the latest research in mental health and hospice care practices to continuously enhance your clinical skills and knowledge.

You will primarily work with patients facing end-of-life challenges, creating individualized care plans that address not only their mental health needs but also their emotional and familial dynamics. To be considered for this role, candidates must possess:

• A Master's degree in Social Work from an accredited institution.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Florida.
• At least 3 years of clinical experience, ideally within mental health or hospice care settings, demonstrating proficiency in psychotherapy.
