• Conduct comprehensive video intake assessments to evaluate new members' symptoms, functional limitations, and daily habits, setting the groundwork for effective therapeutic interventions.
• Design individual treatment programs that seamlessly integrated into the daily lives of members, including practical strategies to manage symptoms of anxiety, depression, and trauma.
• Provide ongoing support through our digital platform, offering instant encouragement, psychoeducation, and revisions to care plans based on patient feedback and progress.
• Analyze engagement data and perform clinical assessments to adapt care plans that optimize member outcomes and enhance their quality of life.
• Celebrate achievements with members, while collaboratively identifying and addressing barriers to engagement, ensuring continued progress in their health journey.
• Maintain accurate, confidential clinical records in compliance with HIPAA and industry standards, ensuring that all documentation is thorough and up-to-date.
• Collaborate closely with a multidisciplinary team, including psychiatrists and healthcare providers, to guarantee integrated and holistic care for all members.
• Participate actively in case reviews, team meetings, and professional development opportunities to contribute to a culture of continuous learning and improvement.

Our primary focus is on serving individuals in hospice care, including patients grappling with anxiety, depression, substance dependencies, and trauma. Many of our clients are in critical transitions in their lives, often dealing with end-of-life issues. The successful candidate will be sensitive to the complex emotional needs of these patients and their families, providing a safe space for discussions about fears, desires, and life transitions.

• A master's degree in social work or a doctorate in the field, with a current state license in good standing.
• Minimum of two years’ clinical social work experience, holding an LCSW or higher supervision credential.
• Strong familiarity with clinical best practices and therapeutic interventions tailored to individual needs, specifically within the context of hospice care.
• Exceptional written and verbal communication abilities, with a focus on building relationships and rapport with patients.
• A collaborative spirit that aligns with team-oriented care strategies, actively contributing to multidisciplinary discussions.

• Experience with simulation-based education methods to enhance patient learning.
