What Does a Remote Diabetes Educator Do?

Remote diabetes educators provide clients or the general public with education and information about diabetes. Instead of working in the office, remote diabetes educators work from home or another location outside of the office, though they may travel to sites to conduct classes or lectures. As a remote diabetes educator, you may work with patients one-on-one, either on the phone or through virtual portals, to identify effective ways to manage the disease, how to cope with the difficulties associated with it, and assist them in finding public resources available to them. You may also organize informational seminars and create pamphlets for community events.

What are the key skills and qualifications needed to thrive as a Remote Diabetes Educator, and why are they important?

To thrive as a Remote Diabetes Educator, you need a thorough understanding of diabetes management, patient education techniques, and typically hold a healthcare degree with certifications such as Certified Diabetes Care and Education Specialist (CDCES). Experience with telehealth platforms, electronic health records (EHRs), and digital communication tools is often required. Outstanding interpersonal skills, patience, and the ability to communicate complex information clearly help build trust and support patient engagement remotely. These competencies are essential for empowering patients to manage diabetes effectively while ensuring seamless and personalized care in a virtual environment.

How does a Remote Diabetes Educator collaborate with other healthcare professionals to support patient care?

As a Remote Diabetes Educator, you'll regularly coordinate with physicians, nurses, dietitians, and pharmacists to develop and adjust individualized care plans for patients. Communication often occurs via secure messaging platforms, telehealth conferences, and electronic health records. This collaborative approach ensures that patient progress, medication changes, and lifestyle recommendations are aligned across the care team, providing comprehensive and consistent diabetes management. Effective teamwork and proactive communication are essential to address patient needs promptly and optimize health outcomes.

What is a Remote Diabetes Educator?

A Remote Diabetes Educator is a healthcare professional who provides education, support, and guidance to individuals living with diabetes through virtual platforms such as phone calls, video conferencing, or online messaging. Their primary role is to help patients understand their condition, manage blood sugar levels, and adopt healthy lifestyle habits. They may also assist with medication management, meal planning, and addressing patient concerns, all while working remotely. This approach increases accessibility and convenience for patients, especially those in rural or underserved areas.

ABOUT THE ROLE

We are seeking an experienced Nurse Practitioner (NP) passionate about remote care delivery. In this role, you will lead diabetes education and management by conducting phone visits for initial member consultations. You will be responsible for evaluating diagnoses, medications, and lab results to assess member acuity and validate candidacy and continuation in our clinical program. Additionally, you will be identifying and prescribing CGMs (Continuous Glucose Monitor) for applicable members.

KEY RESPONSIBILITIES
  • Provide call-based care to members in Perry Health's Remote Patient Monitoring (RPM) program.
  • Deliver swift, empathic care to members and assess their clinical competencies.
  • Evaluate medical history and data to determine member eligibility for the clinical program.
  • Serve as the primary clinical point of contact for practice-wide escalations.
  • Collaborate with the broader care team โ€” including RNs, RDs, and Accountability Coaches โ€” to reinforce clinical goals and SMART goal progression.
  • Adhere to Perry Health policies, procedures, and standard operating procedures to ensure ongoing clinical quality and compliance.

  • Skills: Motivated learner, willing to acquire proficiency with the necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to support sustainable behavior change is a plus.
  • Traits: Creative thinking, an eye for detail, ability to work independently, a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
  • Language: Spanish-speaking highly preferred.
COMPENSATION & PAYMENT TERMS

Contractors are paid on a monthly basis following Net 30 payment terms.

Clinical Pay: $30.00 per completed consult. Consults are scheduled in 20-minute slots and require a minimum of 15 minutes of engagement time per member โ€” inclusive of live conversation, chart review, and care coordination โ€” for the visit to be billable. Payment is issued for completed, billable visits.

Non-Clinical Pay: $30.00/hour flat rate for required meetings and training.
