Job description

Overview

The HIM Data Integrity Specialist is responsible for ensuring the accuracy, completeness, and integrity of patient health information within the Epic Electronic Medical Record (EMR) system. This role supports data quality initiatives by identifying, analyzing, and resolving data discrepancies such as duplicate medical records, overlays demographic inconsistencies and erroneous documentation. The specialist collaborates with clinical and administrative teams to support patient safety, regulatory compliance, and optimal system performance while promoting best practices in health information management.

Responsibilities

Essential Functions

  • Resolve and merge duplicate medical records identified.
  • Verifies and resolves patient overlays.
  • Maintain and manage work queues related to data integrity issues, ensuring timely resolution in accordance with departmental standards using Epic tools.
  • Add appropriate chart flags or comments as needed to ensure integrity of the medical record.
  • Collaborate with Patient Access, Registration, Clinical Staff, and other teams to correct errors and prevent future data integrity issues.
  • Meets set productivity standards.
  • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including HIPPA, The Joint Commission and CommUnityCare
  • Standard Operating Procedures and CommUnityCare Policies and Procedures.
  • Utilize Epic reporting tools and dashboards to monitor trends, identify root causes, and recommend process improvements.
  • Provide a high level of customer service to all employees within the organization.
  • Establish and ensure favorable relationships throughout the organization.
  • Ensure all actions, job performance, personal conduct and communications always represent CommUnityCare in a highly professional manner.
  • Maintain confidentiality of sensitive information and data.
  • Maintain up to date knowledge of confidentiality and privacy laws.
  • Develop and maintain strong and favorable internal and external relationships, partnerships with Community Care
  • Collaborative, co-workers, including clinical managers, clinical support staff, providers and business office staff and other partners.
  • Performs other duties as assigned.

  • High level of critical thinking skills.
  • Strong attention to detail and accuracy. 
  • High level of skill at building relationships and providing excellent customer service. 
  • Knowledge of commonly used medical terminology. 
  • Ability to utilize a computer for data entry and retrieval. 
  • High level of organization and ability to multitask. 
  • Excellent verbal and written communication skills. 
  • Demonstrated ability to perform operations in system EHRS. 
  • Ability to maintain confidentiality. 
  • Ability to manage competing objectives in a fast-moving environment to achieve results.
Qualifications

Minimum Education: 

  • High School Diploma or equivalent (higher degree accepted)

Minimum Experience:

  • 2 years Experience in a healthcare setting.
