Key Responsibilities

In this role, you will be responsible for the following:

  • Conduct comprehensive psychosocial assessments to evaluate patients' mental health needs and develop individualized treatment plans based on these evaluations.
  • Diagnose and treat a variety of mental, emotional, and behavioral disorders while adjusting care plans as necessary to reflect evolving patient statuses.
  • Collaborate with a multidisciplinary team to strategize appropriate discharge plans, ensuring continuous care through referrals to community services, treatment programs, and support groups.
  • Ensure compliance with federal, state, and local mental health standards and regulations while acting within the boundaries of the Mental Health Practice Act.
  • Organize and prioritize your daily workload by effectively managing cases, particularly new, current, and discharging patients, to meet their specific needs.
  • Perform social work consultations for patients and caregivers, delivering crisis interventions, bereavement support, and other necessary psychological services.
  • Communicate and coordinate with patient care teams and external agencies to guarantee timely discharge and resolution of any potential delays in patient care.
  • Act as an advocate for patients and their families, facilitating effective communication with relevant parties concerning treatment plans and available resources.
  • Identify patients at risk for avoidable delays or complex discharge needs and collaborate with care providers to find optimal solutions.
Patient Population / Client Focus

As a Remote Therapy Specialist, you will work primarily with a diverse population of clients experiencing varying levels of mental health challenges. Qualifications
