Key Responsibilities

As a Licensed Mental Health Counselor at Elevance Counseling, your role encompasses a variety of essential tasks, including:

• Conducting in-depth psychosocial assessments to understand each patient's unique challenges and strengths.
• Diagnosing and providing evidence-based treatment for mental, emotional, and behavioral disorders, while regularly updating care plans to reflect changes in patient status.
• Facilitating referrals to relevant community resources, treatment programs, and support groups to enhance your patients' well-being beyond our clinic.
• Collaborating with healthcare providers, families, and community agencies to develop comprehensive discharge plans that ensure continuity of care.
• Adhering to all applicable federal, state, and facility regulations, maintaining compliance with mental health practice standards.
• Managing and prioritizing a diverse caseload by assessing the specific needs of new, current, and discharging patients.
• Providing crisis intervention and bereavement support, employing various therapeutic modalities as needed.
• Advocating for your patients and their families to foster effective collaboration with care teams and community resources.

Patient Population / Client Focus

The clients you will serve include individuals facing a spectrum of mental health issues ranging from anxiety and depression to complex trauma and relationship challenges. Qualifications

• A Master’s Degree in Social Work or an equivalent field from an accredited institution.
• Current licensure in Arizona as an LCSW, LMFT, LPC, or Psychologist.
• Demonstrated knowledge of community mental health resources and services.
• Exceptional interpersonal, written, and verbal communication skills, with the ability to establish rapport with diverse client populations.

Preferred Experience

• Experience in a community health environment or similar setting.
• Familiarity with electronic health record systems and telehealth practices.
• Previous involvement in multi-disciplinary team collaboration and care coordination.

Compensation and Benefits
