Key Responsibilities
  • Proactively engage with participants to prevent the need for crisis services by providing early intervention strategies.
  • Develop, complete, and maintain crisis alerts to streamline coordination with county crisis services for participants, ensuring timely support.
  • Facilitate transitions for clients requiring hospital or inpatient services by coordinating with healthcare professionals to support safe discharges.
  • Document all interactions and progress comprehensively, maintaining accurate records to promote continuity of care.
  • Employ evidence-based therapeutic techniques to address psychiatric, social, and emotional challenges faced by clients.
Patient Population / Client Focus

In this role, you will work with a diverse range of patients coping with various mental health issues, including depression, anxiety, substance abuse, and relationship difficulties. Your ability to rapidly establish rapport and connect with individuals from different cultural, linguistic, and socio-economic backgrounds is essential for promoting effective treatment outcomes.

Qualifications
  • A Masterโ€™s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.
  • An active and valid license as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).
