Position Overview

Core Mental Health and Behavioral Solutions is seeking a dedicated and energetic Licensed Clinical Social Worker (LCSW) to join our team in providing compassionate support to patients within a hospice care environment. Key Responsibilities
  • Provide compassionate support to patients and their families by conducting thorough assessments of emotional, mental, and psychosocial needs.
  • Develop personalized treatment plans, setting measurable goals to assist patients in achieving improved mental health outcomes.
  • Engage in therapeutic conversations and interventions to support patients in coping with the emotional impact of their illnesses.
  • Regularly communicate with medical and interdisciplinary teams to coordinate care and monitor patient progress.
  • Document all patient interactions and treatment plans consistently in our electronic health record (EHR) system, adhering to confidentiality standards.
  • Participate in ongoing training and development opportunities to stay current with best practices in hospice care and telehealth.
  • Perform additional duties as assigned to ensure comprehensive patient care and organizational success.
Patient Population / Client Focus

This role specifically addresses the needs of patients in a hospice setting, providing support to individuals of varying ages and backgrounds dealing with terminal illnesses. You will interact directly with patients and their families, offering emotional and psychological support, helping them navigate the complexities of end-of-life care, and ensuring that their needs and preferences are honored throughout the process.

Qualifications
  • A Master’s Degree in Social Work with a current license as a Licensed Clinical Social Worker (LCSW) in California.
  • A minimum of 2 years of direct practice as an LCSW, with a focus on mental health and psychosocial assessments.
  • Ability to provide telehealth services effectively, utilizing appropriate technologies to facilitate connections with patients.
  • Strong knowledge of mental health conditions and various therapeutic modalities suited for hospice care.
Preferred Experience
  • Experience in a hospice or palliative care setting, particularly in providing support to terminally ill patients and their families.
  • Familiarity with electronic health records (EHR) and teletherapy platforms.
  • Previous training in grief counseling or working with bereaved individuals is preferred.
Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, accompanied by a comprehensive benefits package.
