Humana/iCare is looking for an RN Care Manager to join the Dane County team in the Family Care Partnership (FCP) program. Reporting to the Manager of Care Management Support, you will work within an Interdisciplinary Team (IDT) setting to help manage members that are enrolled in the Family Care Partnership program. You will conduct home and community visits to members to complete the nursing assessment and assures members have appropriate care and access to services.

Responsibilities:

  • Complete the nursing portion of the comprehensive assessment and provide input to the members care accordingly.
  • Establish and monitor wellness and health promotion activities based on the member's assessment and identified outcomes.
  • Conduct regular assessments of members' health and wellness to manage ongoing conditions, monitor the effectiveness of care interventions, and promptly address any new medical concerns or events affecting their well-being.
  • Perform routine health monitoring activities
  • Build constructive working relationships with members, their family members, and appropriate outside agency staff.
  • In crisis situations, provides appropriate assessment of psychiatric, behavioral or medical problems and implements interventions with other staff to take appropriate measures to ensure the safety of members.
  • Evaluate members' ability to safely manage, either by themselves or through use of supports, their medication, medical care interventions and treatments, equipment, and access to health care services.
  • Provide or arrange for services, supports or medical care that best meet the member's identified needs, and monitor the effectiveness of these interventions.
  • Help with program and procedure development and assists with input for the QI/QA program.

Required Qualifications

  • Must be a Registered Nurse, licensed in the State of Wisconsin.
  • Two (2) years of related health care experience.
  • Travel to any location within Dane County and surrounding areas (Sauk)

Preferred Qualifications

  • Bachelor of Science degree in Nursing (BSN).
  • Previous home health, managed care or case management experience.
  • Previous experience working with persons with physical, intellectual/developmental disabilities, or older adults in a health care setting.

Additional Information

  • Workstyle: This is a field position where employees perform their core duties at non-company locations, such as providing services at business partner facilities or prospects' and members' homes.
  • Travel: up to 50% throughout primarily Dane county and surrounding areas
  • Typical Work Days/Hours: Monday - Friday, 8:30 am - 5:00 pm Central Standard Time (CST).

WAH Requirements

To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:

  • At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested.
  • Satellite, cellular and microwave connection can be used only if approved by leadership.
  • Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
  • Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Driving

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

Mileage reimbursement is provided for work-related travel. Eligible mileage includes:

  • Travel from your home to your first work location of the day.
  • Travel between client or assignment locations during the workday.
  • Travel from your final work location back to your home.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
