Key Responsibilities
  • Conduct comprehensive psychosocial assessments of seniors, evaluating emotional, mental, and social needs to inform care planning.
  • Collaborate closely with clients and their families to develop, implement, and regularly review personalized care plans tailored to individual goals and preferences.
  • Provide counseling and emotional support to help seniors and their families navigate significant life transitions or challenges, such as health crises or significant losses.
  • Advocate for clients' rights within community settings, ensuring access to necessary services and support systems to enhance their overall well-being.
  • Engage in interdisciplinary collaboration, working alongside nurses, healthcare professionals, and caregivers to coordinate integrated care that addresses all aspects of a client's needs.
  • Conduct regular check-ins with clients, assessing changes in their living conditions, emotional state, and overall safety.

Qualifications
