About the Opportunity

Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. The Referral Coordinator serves as the scheduling backbone of Pair Team's care coordination model, working behind the scenes to ensure patients receive timely, coordinated care through effective management of all referral and scheduling processes. This role collaborates closely with Case Managers, providers, and health plans to ensure smooth transitions between services while maintaining clear communication and HIPAA compliance throughout.

This role is 100% remote; however, candidates must be based in the United States. Please note that we currently require all team members in this role to work PST hours.

What You'll Do:

Own key Ops Support initiatives

  • Process, track, and coordinate patient referrals to ensure timely scheduling of appointments
  • Schedule PCP, dental, vision, specialist, and transportation appointments for patients
  • Verify patient insurance eligibility and confirm patient's in-network PCP
  • Coordinate Release of Information (ROI) and Authorized Representative (AR) requests
  • Maintain accurate documentation of communications and referral activities in internal systems
  • Collaborate cross-functionally with care team members to support continuity of care and high-quality patient experiences
  • Contribute to continuous process improvements in referral workflows and scheduling efficiency
What You'll Need:
  • Fluency in English and Spanish
  • 1+ years of healthcare operations or administrative experience in a fast-paced healthcare environment
  • Previous experience as a Referral Coordinator, Medical Assistant, or Healthcare Appointment Scheduling experience preferred
  • Strong organizational skills and attention to detail with experience managing multiple tasks and systems simultaneously
  • Comfort navigating insurance portals and verifying eligibility or provider information
  • Excellent written and verbal communication skills, with the ability to maintain professionalism and empathy across all interactions
  • Proficiency with technology such as Google Suite, Slack, and data tools (e.g., Looker or comparable platforms)
  • Reliable, HIPAA-compliant workspace with stable internet connection
  • A commitment to improving healthcare access for underserved communities
