Key Responsibilities
  • Conduct comprehensive assessments to address the mental health needs of patients while developing personalized treatment plans.
  • Actively communicate with supervisors, family members, and other professionals involved in patient care to discuss treatment progress and address any challenges faced by clients.
  • Manage and provide a structured schedule of your sessions, home visits, and follow-ups to ensure consistent support and therapeutic interventions.
  • Coordinating and executing home visits and observations, ensuring that necessary chart reviews and staff interviews take place to deliver effective care.
  • Implement and monitor treatment plans, adapting strategies as required to achieve optimal patient outcomes.
  • Engage in regular interdisciplinary team meetings to share insights, receive feedback, and collaborate on treatment strategies.
  • Document monthly and quarterly progress notes, maintaining a clear and organized record of each patientโ€™s journey.
  • Lead training sessions for staff to enhance their competencies in program implementation and documentation practices.
  • Provide continuous engagement through follow-up interviews and additional home visits to support clients in their therapeutic journey.
  • Liaise with community partners and mental health organizations to foster community outreach and support growth initiatives.
Patient Population / Client Focus

Our counselors work with a diverse range of clients, including adults and youth experiencing various mental health challenges. You will have the opportunity to support individuals dealing with trauma, substance misuse, and family dynamics, utilizing evidence-based therapeutic approaches tailored to their specific needs.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Must hold an active and unrestricted license as a Clinical Professional Counselor (LCPC) in the state of Illinois.
  • At least two years of professional experience in a counseling role, preferably within a clinical setting, is necessary.
  • Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly desirable.
  • Strong knowledge of legal and ethical practices in mental health counseling.
