• Collaborate with primary care practitioners to identify, treat, and manage mental health and psychosocial issues, enhancing the overall effectiveness of the healthcare team.
• Conduct thorough patient intakes that focus on diagnostic and functional evaluations, subsequently developing therapy objectives tailored to individual needs.
• Provide immediate and appropriate interventions for clients requiring urgent mental health support.
• Advise primary care providers on the appropriate treatment paths for patients, indicating those who may benefit from specialized mental health services.
• Perform regular follow-up assessments to monitor patient progress, adjusting treatment plans as needed to optimize outcomes.
• Work collaboratively with patients to create effective relapse prevention strategies, fostering ongoing support and recovery.
• Identify patients at risk of deterioration and contribute to the design of preventive programs tailored to their needs.
• Coordinate and oversee the delivery of mental health services, working within the primary care framework while also engaging with external providers when necessary.
• Maintain accurate and current documentation of patient progress and diagnostic information in accordance with agency standards and compliance regulations.
• Engage in consistent communication with primary care providers, ensuring they are informed of patients’ needs and treatment progress, and participate in joint treatment planning.
• Empower patients to make positive lifestyle changes and assist them in managing their treatment regimens, including addressing any medication side effects.
• Consult with clinic administration to explore opportunities for service improvement, outcome evaluation, and program development.
• Adhere to the Clinical Professionals' Code of Ethics and maintain a professional demeanor within all interactions.
• Perform additional duties as assigned to support team objectives.

• A Master’s Degree in Clinical Counseling is mandatory.
• A valid Licensed Professional Counselor (LPC) license is required.
• Proficiency in mental health evaluation, treatment planning, developmental psychopathology, and various therapeutic methodologies.
• Understanding of the community resources available to address both physical and mental health needs.
• Knowledge of psychodiagnostics and the application of evidence-based therapeutic practices.
• Fluency in English is essential; proficiency in Spanish is a desirable asset but not mandatory.
• Robust capacity to manage the demands of counseling while maintaining personal well-being.

Experience working within a clinical setting or community health environment is preferred, particularly with a focus on integrated care models that combine primary and mental health services. Familiarity with telehealth platforms, electronic health record (EHR) systems, and an understanding of insurance reimbursement processes, including billing for community health clinics, will be advantageous.
