Key Responsibilities

As a Clinical Social Worker, you will carry significant responsibilities that directly impact patient health outcomes. Your day-to-day tasks will include:

• Conducting thorough bio-psychosocial assessments tailored to the mental health, substance abuse, and medical needs of patients within a primary care context.
• Providing individual and group counseling sessions, performing intake evaluations, and developing personalized treatment plans based on each client’s unique circumstances.
• Utilizing evidence-based practices for crisis intervention, ensuring a swift response to acute patient needs, including potential referrals for substance use treatment.
• Documenting patient interactions diligently, maintaining comprehensive electronic health records that accurately capture progress notes, treatment updates, and assessments.
• Acting as a liaison for patients, offering social work support and connecting them with community resources, including assistance programs for medication.
• Participating in quality improvement initiatives, contributing to data collection efforts that improve our overall service delivery and patient care processes.

Qualifications

The ideal candidate for this role will possess the following qualifications:

• A valid state license as a Licensed Clinical Social Worker (LCSW) is required.
• A Master’s degree in Social Work.
• A minimum of three years of post-licensure experience providing clinical social work services.

Compensation and Benefits
