Key Responsibilities

• Conduct thorough biopsychosocial assessments tailored to each client and their unique circumstances, documenting findings electronically to ensure accuracy and compliance.
• Provide individualized therapy and counseling services, including crisis intervention, treatment planning, and ongoing case management for clients across various demographics.
• Facilitate both individual and group counseling sessions, employing evidence-based methods to engage clients in meaningful discussions aimed at fostering long-term change.
• Escalate patient needs and concerns to appropriate healthcare providers when necessary, advocating for clients while ensuring comprehensive care pathways.
• Collaborate with Quality Improvement teams to refine and develop best practices for data collection and service delivery, enhancing patient care outcomes.
• Act as a resource for social work support, building bridges to local resources and community agencies to provide clients with access to medication assistance programs and additional services.

Patient Population / Client Focus

In this role, you will serve a diverse population of children, families, and individuals facing an array of challenges including mental health issues, substance use disorders, and socioeconomic barriers. Your interventions will not only provide them with necessary treatment and support but will also empower them to achieve greater stability in their lives, thus enhancing overall community health.

Qualifications

• A Master's degree in Social Work is required, along with an active state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of three years of post-licensure experience in a clinical setting, demonstrating case management and counseling skills.
• Demonstrated proficiency in crisis intervention techniques and therapeutic methods, with the ability to exercise independent judgment in delivering services.
• Strong communication skills and a competency in group facilitation to effectively engage clients and support community interactions.

Preferred Experience

• Experience working in integrated healthcare settings, particularly those focused on holistic approaches to mental and physical health.
• Familiarity with remote therapy and telehealth platforms, ensuring effective delivery of services in a virtual environment.
• A history of collaboration with multidisciplinary teams to create comprehensive care plans that address all aspects of client health.
