Key Responsibilities
  • Evaluate, diagnose, and treat a diverse range of psychosocial problems through the application of clinical social work skills.
  • Collaborate with medical and support staff to develop comprehensive care plans tailored to individual patient needs, increasing treatment compliance and care coordination.
  • Conduct bio-psychosocial evaluations to address barriers to treatment, enhance patient health, and optimize resource utilization.
  • Provide both individual and group therapies, employing evidence-based treatment modalities to maximize patient outcomes.
  • Document all social work activities diligently in patients' medical records, adhering to departmental standards.
  • Offer education and support to patients and families, helping them understand and navigate medical recommendations and social services.
  • Assess advocacy needs and coordinate appropriate community resources focused on enhancing social support and self-determination.
  • Maintain adherence to NASW Code of Ethics and corporate values, ensuring a safe and compliant environment of care.
  • Stay current on community resources and educational opportunities to continually enhance professional expertise.
  • Contribute to departmental reports and case-related statistics, helping to shape the ongoing development of the facility's services.
Patient Population / Client Focus

Your role will engage with a diverse patient population, including children, adolescents, adults, and their families, who are facing challenges stemming from mental health issues, trauma, or chronic illnesses. You will tailor your interventions to consider the cultural patterns and specific needs of the patients you serve, pursuing equitable and empathetic treatment strategies.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution and a current license from the State Board of Social Work Licensure, specifically at the LCSW level.
  • Demonstrated knowledge of human development, aging processes, and cultural competence in delivering care.
  • Strong analytical skills to evaluate patients and develop effective treatment plans.
Preferred Experience
  • 1+ years of relevant behavioral health experience, particularly in outpatient settings or with hospice care, is highly desired.

Compensation and Benefits

The salary range for this position is competitive, ranging from USD 85,000 to 100,000 annually, commensurate with experience.
