Patient Outreach Specialist: Remote Healthcare Position

ABOUT GRAPEFRUIT HEALTH
Grapefruit Health is a healthcare services company on a mission to solve the growing staffing shortage in healthcare by putting tomorrow's clinicians to work today. We train and manage healthcare graduates to conduct remote, telephonic patient interactions using our purpose-built software. The result: patients receive timely support, healthcare organizations extend their capacity, and you gain real clinical experience.
THE ROLE
As a GFH Patient Outreach Specialist, you'll conduct structured phone-based interactions with patients on behalf of our partner healthcare organizations. Using our software, you'll follow evidence-informed protocols to check in on patients, complete assessments, and escalate concerns when needed, all from a quiet space of your choosing. You'll work with patients experiencing different disease states, preventative care needs, medication use, post-discharge follow-up needs, etc. All interactions are telephonic; no video, no in-person contact required. Must be based in the U.S.
WHAT YOU'LL DO
  • Complete a minimum of 20 hours of patient outreach per week on a flexible schedule you build
  • Conduct structured telephonic assessments and wellness check-ins using The Grove (Grapefruit Health's platform)
  • Document interaction outcomes accurately within the platform
  • Escalate patient safety concerns to a licensed clinical supervisor in real time
  • Maintain strict HIPAA compliance and patient confidentiality at all times
  • Participate in onboarding training and ongoing check-ins
TRAINING & SUPERVISION
All new graduate hires complete a ≦14 day paid onboarding covering our platform, communication frameworks, and HIPAA requirements. Once active, you'll be supervised by our Clinical Leadership Team who are available during your shifts for escalations and support.

WHAT WE'RE LOOKING FOR
  • U.S. based required
  • Must be a healthcare school graduate - active clinical enrollment does not qualify for this role. You will be rejected if you are in school
  • Certification as a medical assistant, community health worker, patient navigator, pharmacy technician, or comparable clinical credential is a plus
  • Must have a compatible computer and cell phone with SMS
  • Must be able to work at least 20 hours/week
  • A quiet, private workspace free of background noise or distractions
  • Strong verbal communication skills, or a genuine desire to develop them
  • Comfort working independently in a remote environment
