As a Remote Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive clinical evaluations of patients and their families, assessing emotional and psychological needs based on hospice care parameters.
• Formulating individualized treatment plans that incorporate evidence-based practices, utilizing interventions such as cognitive-behavioral therapy (CBT) and solution-focused therapy.
• Maintaining accurate clinical documentation in compliance with HIPAA standards while ensuring timely communication with other team members.
• Collaborating with interdisciplinary teams, including nurses, physicians, and other healthcare providers, to ensure a seamless care experience for patients.
• Participating in ongoing case consultations and peer reviews, engaging in professional development to enhance care delivery.
• Educating patients and families on mental health resources, coping strategies, and available community services pertinent to their unique situations.
• Staying updated on the latest research and innovations in hospice-related mental health treatment to continually improve practice.

Your clients will primarily include patients in hospice care and their families, who face diverse and often complex emotional challenges related to end-of-life issues. This requires a deep understanding of grief, loss, and trauma, as well as the ability to provide empathetic support and practical solutions for coping with these experiences. You will be instrumental in helping clients navigate their feelings and embrace a supportive treatment approach as they journey through advanced illness.

The ideal candidate will possess the following qualifications:

• A Master of Social Work (MSW) degree from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the relevant state.
• A minimum of two years of professional experience providing psychotherapy services, with a preference for experience in hospice or palliative care environments.
• Strong proficiency in therapeutic interventions, particularly solution-focused therapy.
• Excellent communication and interpersonal skills that foster trust and rapport with patients and families.
• Ability to think critically and solve problems effectively in a team-oriented setting.

While not required, experience in similar settings such as hospice, home health, or interdisciplinary medical teams is desirable. Familiarity with the well-being of seriously ill patients and their families can enhance your success in this role.
