• Conduct thorough psychosocial assessments to evaluate each patient's emotional, social, and financial needs, ensuring a holistic approach to their care.
• Provide individual and group counseling sessions to patients, addressing issues such as grief, adjustment, and coping mechanisms rooted in their recovery journey.
• Collaborate closely with medical teams, including physicians, nurses, and addiction specialists, to develop cohesive treatment plans and optimize patient outcomes.
• Coordinate with community organizations and resources to arrange post-discharge services, identifying placement needs that facilitate ongoing recovery.
• Engage in crisis intervention initiatives, offering timely emotional support to patients and families in distress.
• Participate actively in hospital committees, contributing your expertise on social work matters and advocating for the integration of mental health in treatment protocols.
• Maintain comprehensive documentation that adheres to hospital policies and legal requirements, ensuring transparency and continuity of care.
• Develop and implement effective discharge plans, connecting patients with appropriate community resources to support their recovery.

• A Master's degree in Social Work from an accredited program.
