This role is primarily remote, allowing for flexibility in your working hours while supporting clients through video conferencing and digital communication tools. As an integral member of the hospice team, your contribution will directly impact the quality of care provided to our clients.

• Develop comprehensive, individualized treatment plans based on thorough client assessments, which may involve both individual and group therapy sessions.
• Deliver high-quality therapy sessions that utilize evidence-based practices aimed at helping clients overcome dependencies and foster positive changes in their lives.
• Collaborate with clinical leadership in conducting chart audits, ensuring that all documentation aligns with ethical and legal standards.
• Maintain meticulous records of client progress, treatment plans, and evaluations in compliance with state and federal regulations.
• Facilitate team meetings and participate in multi-disciplinary case consultations to optimize treatment outcomes.

• A Master’s degree from an accredited institution in Social Work, Psychology, Counseling, or a related field.
• Active state license as a Licensed Clinical Social Worker (LCSW).

• 2-3 years of direct experience working in a social work capacity in mental health or addiction recovery environments.
• Demonstrated knowledge of case management and support services that can enhance patient care.
• Proficiency with electronic health records (EHR) and telehealth platforms to facilitate remote care delivery.
• Strong interpersonal skills, with the ability to develop therapeutic relationships with clients.
• Exceptional written and verbal communication skills for effective documentation and collaboration.

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience.
