• Act as a liaison between clients, families, and outpatient providers ensuring cohesive communication throughout the care continuum.
• Engage with insurance providers for pre-certification and manage utilization effectively to optimize patient care.
• Participate actively in discharge and aftercare planning, ensuring each client is prepared for the next steps in their care journey.
• Provide therapeutic meal support, guiding clients toward developing a healthier relationship with nutrition and food.
• Deliver empathetic, empowering, and individualized care, considering the unique circumstances, cultures, and identities of each client.

This role focuses specifically on older adults receiving hospice care. You will work with patients facing complex emotional and psychological challenges during a sensitive stage of their lives. Understanding their unique experiences, aspirations, and intersecting identities will be vital to ensuring they receive the best possible care tailored to their needs. Your role will directly contribute to enhancing their emotional well-being and overall quality of life during this critical time.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of California is mandatory for this position.
• Experience working with older adults, particularly in hospice or palliative care settings, is essential.
• Strong clinical skills and diagnostic abilities, with a focus on compassionate patient communication.
• Proven conflict resolution abilities and interpersonal skills to build trust with clients and their families.
• Ability to work independently, as well as collaboratively within a multidisciplinary team.

• Experience utilizing electronic health records (EHR) systems for documentation and care management.
• Familiarity with crisis intervention techniques and trauma-informed care approaches.
• Previous experience in a hospice or home healthcare setting, providing direct support to patients and families.

The compensation for this role ranges between USD 115,000 and USD 130,000 annually, commensurate with experience and qualifications.
