Kaleidoscope Healing for Mental Health, LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity within a Hospice Center setting. This role is pivotal in providing high-quality behavioral health support to patients at a critical juncture in their lives. Key Responsibilities
  • Conduct thorough biopsychosocial assessments to evaluate mental health, substance use, and medical needs for patients within the hospice care context.
  • Facilitate individual and family counseling sessions that promote emotional healing and coping strategies, using evidence-based practices tailored to patient needs.
  • Document and report progress notes, treatment plans, and return for revisions or updates as needed, ensuring compliance with federal and state regulations.
  • Provide support in crisis situations, employing strong intervention techniques to stabilize patients and recommend appropriate courses of action, including referrals to community resources.
  • Collaborate with multidisciplinary teams to execute quality improvement initiatives and improve data collection processes for better patient outcomes.
Patient Population / Client Focus

You will primarily serve patients and their families within the hospice care setting, focusing on those experiencing terminal illnesses. Your role will encompass the unique emotional and psychological challenges these patients face, as well as addressing the needs of their loved ones as they navigate this sensitive phase of life. Qualifications
  • Active Licensed Clinical Social Worker (LCSW) certification in the State of California.
  • A Master’s degree in Social Work from an accredited institution.
  • A minimum of three (3) years of post-licensure clinical experience, preferably in a healthcare or behavioral health setting.
Preferred Experience

Compensation and Benefits
