Berks Community Health Center is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our interdisciplinary team. This full-time, remote position offers a salary ranging from $90,000 to $100,000 annually. • Establish rapport with patients, their families, and treatment team members, utilizing active listening and clear communication tailored to individual needs.
• Function as an integral member of an interdisciplinary team, collaborating with behavioral health, primary care providers, and other specialists to provide comprehensive patient care.
• Conduct brief, evidence-based screenings for mental health conditions and facilitate access to comprehensive assessments as necessary, addressing issues such as depression, anxiety, and substance abuse.
• Manage a personal caseload, providing brief interventions and follow-up care to ensure ongoing support for patients’ mental health needs.
• Document all patient interactions and care activities in the electronic medical record system within 48 hours to maintain accurate and up-to-date patient information.
• Develop, implement, and monitor individualized care plans that align with the patient's needs and link them to necessary services, while ensuring effective communication among all treatment team members.
• Safeguard patient privacy and confidentiality in all communications, documentation, and data handling.
• Engage in supervision, case conferences, and departmental meetings to enhance team collaboration and quality improvement practices.
• Provide referrals for prescribed psychiatric medications and connect patients with community resources that support their overall goals and interests.
• Return calls and follow up with patients, families, and other providers to address concerns or schedule appointments.

This role focuses primarily on serving underserved communities, including individuals experiencing homelessness and those facing systemic barriers to healthcare. A strong understanding of the cultural, social, and economic challenges these patients encounter will be vital in providing empathetic and tailored support.

• A Master’s degree in a related Behavioral Health field is required.
• Possession of an active Licensed Clinical Social Worker (LCSW) credential.
• Certification in CPR or BLS is preferred.
• A minimum of 3-5 years of experience in the behavioral health field, with a strong focus on clinical social work.
• Demonstrated cultural competence and the ability to work effectively with BCHC’s target populations.
• Experience employing trauma-informed care and a social justice framework in clinical practice.

• Bilingual proficiency in English and Spanish is highly advantageous.
• Prior experience working in integrated healthcare settings, ideally with a focus on both behavioral and physical health.
• Familiarity with electronic medical record systems and data management practices.
