This full-time role allows for a flexible remote work setting, making it easier for you to balance your personal and professional life while providing essential support to our hospice clients in Allentown, Pennsylvania. As a team member, you will collaborate seamlessly with other healthcare professionals, ensuring that our clients receive consistent and comprehensive care.

• Conduct thorough mental health assessments, including psychosocial evaluations and risk assessments, to guide your treatment approach.
• Collaborate with patients to craft individualized treatment plans that take their specific needs, cultural backgrounds, and personal goals into account.
• Utilize evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), in individual and group settings.
• Provide crisis intervention services, coordinating care with other healthcare professionals to manage acute patient distress effectively.
• Maintain accurate clinical documentation, including progress notes and treatment plans, ensuring compliance with regulatory guidelines and clinic policies.
• Conduct psychoeducation sessions with patients and their families to empower them to effectively manage mental health issues.
• Engage in ongoing outcome assessments, reviewing treatment efficacy and adapting plans as necessary based on patient feedback and clinical data.
• Stay updated on current mental health research and trends, integrating new findings into your clinical practice.
• Ensure confidentiality and adhere strictly to ethical guidelines when handling patient information.

The social worker will focus primarily on hospice patients, many of whom are coping with end-of-life issues and complex emotional challenges. This role will require not only clinical expertise but also deep empathy and understanding to offer both patients and their families the emotional and psychological support they need during this profound time in their lives.

• A Master’s degree in Social Work is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• A proven ability to work both independently and collaboratively within a multidisciplinary care team.
• Exceptional interpersonal skills, with the ability to build rapport and trust with a diverse clientele.
• Strong communication skills, both verbal and written, to deliver clear and compassionate care.

• Experience in hospice or palliative care settings is highly desirable.
• Familiarity with trauma-informed approaches and evidence-based therapeutic practices.
• A track record of effective case management and patient advocacy.

• The annual salary for this position ranges from $90,000 to $100,000, commensurate with experience and qualifications.
