Key Responsibilities
  • Conduct individualized therapy sessions via telehealth platforms, guiding patients through their therapy journey.
  • Develop and implement personalized treatment plans tailored to the unique needs of each patient, based on thorough intake assessments and ongoing evaluations.
  • Engage in crisis intervention when necessary, employing evidence-based techniques to support patients in high-stress situations.
  • Maintain accurate and comprehensive progress notes in adherence to HIPAA guidelines, documenting key observations and treatment adjustments.
  • Analyze data related to patient progress and treatment outcomes, utilizing findings to refine therapeutic approaches and share insights with the broader clinical team.
  • Collaborate closely with medical professionals, providing insights into psychosocial aspects of care and advocating for patients’ holistic needs.
  • Educate patients and families about available resources, coping strategies, and wellness practices that can support ongoing mental health.
Patient Population / Client Focus

In this role, you will primarily work with adults and adolescents facing a range of mental health challenges, including but not limited to depression, anxiety, and coping with grief and loss in a hospice setting. You will utilize your skills to address psychosocial issues associated with chronic illness and end-of-life care, ensuring that patients and their families have access to the support they need during critical moments.

Qualifications
  • A Master’s degree in Social Work from an accredited institution, along with a current LCSW license in Pennsylvania.
  • A minimum of two years of relevant experience working as a Licensed Clinical Social Worker, particularly in mental health settings.
  • Demonstrated proficiency in crisis intervention techniques and familiarity with mental health best practices.
  • Strong interpersonal and communication skills, with a proven ability to connect with diverse populations.
  • Excellent organizational skills to manage caseloads and documentation effectively, ensuring compliance with state and federal regulations.
Preferred Experience
  • Experience providing therapy in a virtual format, utilizing telehealth technologies effectively.
  • Previous work within hospice or palliative care settings is highly desirable.
  • Knowledge of community resources and support services available for patient and family assistance.
Compensation and Benefits

The position offers a competitive annual salary ranging from USD 90,000 to 100,000, reflecting the experience and qualifications of the candidate.
