• Provide individualized counseling and support to patients and their families, guiding them through adjustment issues related to illness or disability, and promoting emotional well-being.
• Collaborate closely with the rehabilitation team to create and adjust personalized treatment plans that take into account patients' psychosocial needs, facilitating a holistic approach to recovery.
• Assist patients in navigating community resources, ensuring they have access to necessary services such as support groups, vocational rehabilitation, and housing assistance, essential for their reintegration into the community.
• Participate actively in discharge planning processes to ensure smooth transitions for patients returning home or moving to different care settings, reducing the risk of readmission.
• Offer education and resources to patients’ families to empower them in understanding available post-rehabilitation services, facilitating ongoing support systems.

The patients you will work with at Central Mind Wellness Centre come from diverse backgrounds, facing various physical and emotional challenges due to injuries, chronic illnesses, and disabilities. • A master's degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

The salary for this role ranges between USD 75,000 and 100,000 annually, with opportunities to earn a sign-on bonus.
