• Conduct comprehensive assessments and ongoing evaluations to tailor counseling strategies to meet each client's needs effectively.

• Engage with families, guardians, and healthcare professionals to discuss individuals' progress and collaboratively devise strategies for continued improvement.

• Develop and maintain a tentative schedule for appointments, home visits, and necessary observations to ensure effective monitoring and support.

• Coordinate and conduct telehealth follow-up visits, documentation, and assessments to sustain support for clients in various settings.

• Ensure that the implementation of care plans is executed accurately, and monitor their effectiveness to achieve desired outcomes.

• Participate actively in interdisciplinary team meetings, contributing insights and receiving feedback regarding patient care plans.

• Document monthly and quarterly progress notes to keep accurate records that inform ongoing treatment strategies.

• Lead training initiatives for staff and team members, ensuring competency in implementing care plans and maintaining appropriate documentation.

• Build community relations as a liaison with public and mental health sectors to support program development and client resource access.

• Assist in enhancing organizational procedures by developing training materials aimed at improving service delivery and staff support.

The successful candidate will work with a diverse range of clients, including both adults and youth, addressing various mental health challenges such as anxiety, depression, trauma, and substance misuse. • A Master's or Doctoral degree in Counseling, Psychology, or a related discipline is required.
