This position is fully remote, offering flexibility in your work environment while maintaining a strong connection with our clients and team members. You will be equipped with the tools and technologies necessary to conduct assessments and provide therapeutic support via phone or video conferencing platforms, ensuring that our clients receive consistent care regardless of geographical barriers.

• Plan, coordinate, and manage comprehensive support plans to assist clients in overcoming difficulties and establishing healthier patterns.
• Conduct thorough interviews and assessments with clients, identifying their current conditions, strengths, needs, and areas for growth.
• Create tailored treatment goals for each client and develop measurable objectives to track their progress.
• Regularly monitor, evaluate, and modify treatment plans to reflect clients' evolving needs and challenges.
• Provide informed counseling and guidance during sessions, discussing potential courses of action that align with their individual circumstances.
• Maintain meticulous records of client interactions, treatment progress, and any referrals made to other services.
• Engage in case conferences, collaborating with multidisciplinary teams to ensure holistic support for clients.
• Act as a vital liaison to community resources and external agencies to facilitate clients' recovery and access to additional services.

Your work will focus on a diverse clientele encompassing individuals coping with a variety of psychosocial challenges. You will provide meaningful support to clients facing critical issues that significantly affect their mental health, thereby directly contributing to improved outcomes and quality of life for vulnerable populations. • A Master’s degree in Social Work (MSW) from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated clinical experience in social work and counseling, particularly within a mental health framework.
