• Conduct comprehensive psychosocial assessments to determine patients' emotional, social, and financial needs, ensuring a tailored approach to each case.
• Provide individual and group counseling sessions to address common issues such as adjustment, grief, and coping strategies, strengthening patients’ mental resilience.
• Collaborate closely with medical teams, including doctors and nurses, to develop holistic treatment plans that prioritize both physical and mental health.
• Coordinate with local community organizations to arrange post-discharge services, ensuring a seamless transition and continuity of care for patients.
• Offer education and resources to patients and their families regarding treatment options, promoting informed decision-making and empowering them in their healthcare journey.
• Participate actively in hospital committees and professional development activities, providing insight from a social work perspective to improve organizational practices.
• Maintain thorough and accurate documentation in compliance with organizational policies and legal standards, facilitating effective communication across all patient care teams.

Your primary focus will be on individuals seeking detoxification services, including those dealing with substance use disorders and mental health challenges. You will engage with a diverse clientele, many from the Latinx community, who may face unique cultural and socio-economic barriers. Understanding and addressing these factors will be key to providing effective support and fostering a safe environment for recovery.

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Proficient in various therapeutic methodologies and culturally competent practices.

• At least two years of experience working in a healthcare or hospital setting, ideally within a detox or addiction treatment context.
• Familiarity with medical terminology, mental health disorders, and proven crisis intervention techniques.
• Strong interpersonal and communication skills, capable of collaborating effectively with colleagues and engaging with patients compassionately.
