Key Responsibilities
  • Conduct thorough, ongoing assessments to identify and address the needs of patients, tailoring interventions accordingly.
  • Maintain effective communication with supervisors, direct care staff, family members, and other stakeholders about patient progress, ensuring collaborative care.
  • Develop and provide a tentative schedule of appointments, home visits, and essential observations for monitoring patient outcomes and care progression.
  • Facilitate and coordinate home visits and in-home observations to gather critical insights for chart reviews and staff interviews.
  • Ensure the effective implementation of care plans, regularly assessing their impact on patient well-being.
  • Participate actively in Interdisciplinary Team Meetings and additional relevant meetings to align on patient-centric goals.
  • Document monthly and quarterly progress notes to track developments and revisions in treatment plans.
  • Organize training and certification for staff to ensure competency with program implementation and documentation requirements.
  • Facilitate follow-up interviews and home visits to sustain support for individuals as per their evolving needs.
  • Maintain transparent records that are readily available and accessible to authorized parties, enhancing continuity of care.
  • Serve as a liaison within the broader mental health community, fostering connections to support developmental goals.
  • Contribute to the creation of procedures and training materials aimed at improving therapeutic programs.
Patient Population / Client Focus
