This role is designed for a remote setting, allowing you to efficiently manage your schedule while being part of a collaborative healthcare team. Regular communication with fellow healthcare providers and participation in team meetings and patient consultations will be key to ensuring coordinated, high-quality care.

• Conduct comprehensive nutrition assessments, taking into account each patient's unique medical history, dietary restrictions, and personal wellness goals.
• Develop and implement individualized nutrition care plans, focusing on practical strategies for diabetes management, including food selection and meal timing.
• Provide evidence-based nutrition counseling tailored to the needs of patients, educating them and their families about optimal dietary choices to prevent complications and promote health.
• Regularly monitor and evaluate patient progress, making necessary adjustments to care plans in collaboration with endocrinologists and certified diabetes educators.
• Engage in nutrition-focused physical examinations and analyze relevant biochemical data to accurately assess nutritional status and needs.
• Foster a patient-centered approach by providing support for lifestyle modifications, including weight management, exercise recommendations, and dietary adjustments based on individual circumstances.
• Maintain meticulous records of patient interactions and nutritional assessments in compliance with relevant healthcare regulations.
• Stay current with developments in nutrition science, integrating new findings into patient care practices to optimize outcomes.

Our clinic primarily serves individuals with diabetes, including type 1 and type 2 diabetes as well as prediabetes. We also cater to patients dealing with related conditions, such as metabolic syndrome and cardiovascular disease, focusing on an overall holistic approach to wellness through nutrition.

• Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution is mandatory.
• Registered Dietitian (RD) credential or Registered Dietitian Nutritionist (RDN) certification from the Commission on Dietetic Registration (CDR).
• Active state licensure or eligibility to practice as a Dietitian/Nutritionist in Oregon.
• Demonstrated expertise in nutrition science, with a strong understanding of medical nutrition therapy as it relates to diabetes management.
• Exceptional communication skills to effectively convey complex nutritional concepts to diverse patients.

• Experience in outpatient settings focusing on diabetes or chronic disease nutrition is highly desirable.
• Familiarity with electronic health record systems to document patient interactions and care plans efficiently.
• Previous collaboration with interdisciplinary healthcare teams, highlighting the ability to work in coordination with physicians, nurses, and diabetes educators.

The position offers an annual salary ranging from USD 50,000 to 70,000, commensurate with experience and qualifications. You will also be eligible for performance-based bonuses, promoting excellence in job performance. Our benefits package includes:
