• Conduct detailed assessments to recognize clients' social, emotional, and environmental challenges related to substance use and mental health.
• Develop individualized service plans that encompass specific goals, interventions, and regularly monitor progress to ensure effective treatment outcomes.
• Provide one-on-one and family counseling aimed at addressing various life challenges associated with addiction, focusing on empowerment and resource access.
• Utilize crisis intervention strategies, including risk assessments and safety planning, to ensure client wellbeing during emergency situations.
• Collaborate with community agencies and organizations to coordinate multiple services, enhancing advocacy efforts for clients to receive the comprehensive support they need.
• Engage in community outreach activities to raise awareness, promote the organization’s services, and strengthen community ties.
• Participate actively in case conferences, team meetings, and ongoing training to remain abreast of best practices and regulatory changes in social work.

As a LICSW at Palm Health Medical Center, you will primarily work with individuals experiencing substance use disorders, along with those confronting related issues such as mental health challenges, trauma, and social adversity. This focused clientele requires understanding, compassion, and a commitment to recovery, as well as the ability to navigate complex social systems to advocate effectively on their behalf.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) in Massachusetts.
• Strong understanding of trauma-informed care principles, social justice issues, and community resources.
• Effective communication and cultural competence to work with diverse populations.
• Excellent problem-solving skills and the ability to adapt to varying client needs and situations.

• Previous experience working within a social service agency, particularly in addiction recovery and mental health settings, is highly valued.
• Demonstrated knowledge and engagement in community resources that support clients in their recovery processes.
• Experience with case management and familiarity with implement best practices for coordinating care across multiple disciplines.
