Job Description:

The Case Manager is an entry-level role that provides administrative and case management support to help ensure members receive timely, coordinated services. Working under the guidance of experienced team members, you'll assist with documentation, service coordination, member communication, and compliance-related activities while gaining valuable hands-on experience in a highly regulated healthcare setting.

This role is ideal for someone who is organized, detail-oriented, enjoys helping others, and is eager to learn and grow within case management.

Key Responsibilities:

  • Support members, families, and service providers through professional communication via phone, email, virtual meetings, and occasional in-person interactions.
  • Assist with coordinating care plans, referrals, follow-up appointments, and service updates.
  • Help maintain accurate case records and documentation within electronic case management systems.
  • Perform data entry, record updates, and administrative tasks while ensuring compliance with organizational and regulatory standards.
  • Track assigned activities, assist with reporting, and support quality assurance initiatives.
  • Collaborate with team members to ensure continuity of services and timely completion of case management activities.
  • Participate in onboarding, training, team meetings, and ongoing professional development.
  • Monitor assigned tasks, escalate concerns when appropriate, and contribute to a positive member experience.
  • Utilize technology and case management software to maintain accurate documentation and workflow efficiency.
  • Perform additional duties as needed to support the overall success of the team.

Qualifications:

Required:

  • Bachelor's degree OR
  • Five (5) years of relevant experience supporting individuals in Long-Term Services and Supports (LTSS), Developmental Disabilities, or a related field OR
  • An equivalent combination of education and applicable experience.
  • Strong customer service and interpersonal communication skills.
  • Excellent organizational skills with strong attention to detail.
  • Ability to prioritize multiple tasks while meeting deadlines.
  • Comfortable using Microsoft 365 and electronic documentation systems.
  • Ability to work both independently and collaboratively in a hybrid work environment.

  • Schedule: Hybrid (remote flexibility offered after training with occasional office and community responsibilities)
  • Employment Type: 6+ month contract with potential for hire
