This position is fully remote, allowing you the flexibility to connect with patients from the comfort of your home while still being an integral part of our dedicated healthcare team. You will collaborate with other healthcare professionals through virtual meetings, ensuring that all patients receive a coordinated approach to their care, despite the physical distance.

• Deliver psychotherapy and mental health services to patients coping with conditions such as anxiety, depression, and PTSD, focusing on their unique psychological needs.
• Conduct comprehensive assessments to evaluate the patients' emotional, behavioral, and social functioning, which will guide treatment plans.
• Develop, implement, and regularly update individualized treatment plans that are evidence-based, culturally sensitive, and specifically tailored to each patient's circumstances.
• Work collaboratively with physicians, psychiatrists, and the hospice care team to ensure a seamless approach to treatment, engaging in regular case reviews and discussions.
• Regularly monitor and assess patient progress, making necessary adjustments to treatment plans based on individual responses and feedback from other healthcare providers.
• Educate patients and their families about mental health conditions, treatment options, and available community resources to support their journey.
• Uphold ethical standards and professional integrity, ensuring patient confidentiality and obtaining informed consent throughout the treatment process.

This role primarily serves patients who are in hospice care, many of whom may be dealing with terminal illnesses or chronic health conditions. The focus is on providing compassionate support, facilitating emotional healing, and helping patients and their families navigate the complexities of end-of-life decisions. Understanding the challenges faced by this sensitive population is key to offering meaningful care and support.

• A Master's degree in Social Work from an accredited institution, ensuring a strong educational foundation in clinical practices.
• Current state licensure as a Licensed Clinical Social Worker (LCSW), verifying proficiency and adherence to professional standards.
• Minimum of 2-3 years of experience in providing mental health services, particularly psychotherapy, to patients with significant health challenges.
• Demonstrated strong clinical skills, particularly in conducting patient assessments and formulating evidence-based treatment plans.
• Excellent communication and interpersonal skills, which are essential for building rapport with patients and collaborating effectively with the interdisciplinary healthcare team.
• Proficient in utilizing electronic health records (EHRs) and various healthcare software systems to streamline patient documentation and care coordination.
• A comprehensive understanding of mental health disorders and the societal factors influencing patient treatment.
• Strong adherence to ethical and professional standards, including maintaining confidentiality and demonstrating integrity in all interactions.

• Experience working specifically within hospice settings or with palliative care patients is highly desirable.
• Familiarity with community health resources and support services that can be beneficial for patients and their families.
• Previous engagement in interdisciplinary teams promoting integrated patient care approaches.
