• Directly engage with clients experiencing a range of mental health challenges, providing tailored therapy in both individual and group settings.
• Conduct thorough assessments and implement evidence-based screening procedures to accurately identify client needs.
• Develop comprehensive integrated care plans that link clients to necessary services and ensure effective communication between all parties involved in a client's care.
• Facilitate crisis intervention strategies and provide immediate support to clients facing acute emotional distress.
• Maintain a manageable caseload, ensuring timely and responsive treatment aligned with clients’ goals.
• Monitor and evaluate client progress, adjusting treatment plans as necessary to meet evolving needs.
• Participate actively in team meetings and collaborate with other healthcare providers to ensure cohesive care delivery.
• Complete all required documentation, case notes, and reports accurately and promptly to support the agency’s quality improvement initiatives.
• Provide on-call coverage as needed and take part in committees focused on enhancing service delivery.

Your work will primarily focus on clients within a hospice setting, many of whom may be facing terminal illnesses alongside complex mental health issues. This patient population requires a specialized approach that emphasizes emotional support, grief counseling, and holistic care, ensuring dignity and respect throughout their treatment process. Understanding cultural sensitivities and adapting to individual client needs are critical components of this role.

• A Bachelor’s degree in Social Work (BSW) is required; a Master’s degree in Social Work (MSW) is preferred.
• Current licensure as a Clinical Social Worker in the state of Colorado.
• Minimum of two years of direct experience in a clinical setting, preferably with hospice or palliative care clients.
• Strong understanding of mental health diagnoses, treatment modalities, and ethical practices in social work.
• Excellent communication and interpersonal skills, with a proven ability to build rapport with diverse individuals.

• Experience working within a remote or telehealth environment.
• Familiarity with integrated care models and interdisciplinary teamwork.
• Proficient in electronic health records (EHR) and case management software.
• Experience in crisis interventions and knowledge of local community resources for client referrals.

The compensation range for this position is between USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
