Job description

A-Line Staffing is now hiring a Care Coordinator II in Kentucky. This position supports a Fortune 500 healthcare company and offers strong career growth potential. This is a full-time, remote opportunity with the possibility of extension and permanent conversion.

Compensation & Benefits

  • Pay: $23.00/hour 
  • Benefits available to full-time employees after 90 days
  • 401(k) with company match available for eligible employees

Schedule Details

  • Remote role — must reside in Kentucky
  • Contract length: ~6 months (extension/convert possible)
  • Training:
    • 2 weeks classroom training + 1 week one-on-one training
    • Camera must be on during training
    • No time off permitted during training
  • Regular hours: Monday–Friday, 9:00 AM – 5:30 PM (EST/CST)
  • Shift flexibility may be required based on business needs
  • Quarterly in-person staff meetings in Louisville or Lexington, KY

Key Responsibilities

  • Conduct outbound member outreach and engagement
  • Help members schedule appointments and access healthcare services
  • Support care coordination and address social determinants of health (SDOH)
  • Educate members on preventive care services and available resources
  • Document all interactions accurately across multiple systems
  • Support outreach initiatives including:
    • Well-child visits
    • Postpartum care
    • A1C testing
    • Breast cancer screenings
  • Maintain productivity, quality, and documentation standards

Requirements

  • High School Diploma/GED
  • 1–2 years of related healthcare experience
  • Experience with member outreach, customer service, or care coordination
  • Experience scheduling appointments and coordinating services
  • Strong data entry/documentation skills; able to navigate multiple systems while on calls
  • Strong communication skills and ability to build rapport quickly
  • Reliable work history with demonstrated longevity
  • Remote work experience preferred
  • Community Health Worker (CHW) certification preferred
  • Knowledge of community resources/healthcare services preferred
