• Collaborate with healthcare team members to develop individualized treatment and discharge plans based on comprehensive psychosocial assessments.
• Conduct thorough assessments and provide interventions for patients dealing with emotional and behavioral challenges, ensuring frequent and precise documentation of clinical notes and progress reports according to departmental protocols.
• Offer counseling services and crisis intervention strategies tailored to the needs of patients, addressing any pressing emotional or behavioral issues they may present.
• Coordinate effectively with patients' families or support networks to create supportive environments that foster successful discharge and ongoing care following their time with us.
• Participate actively in clinical case reviews, contributing insights from social work perspectives to enhance patient outcomes.

The LCSW will work primarily with patients in a hospice setting, providing support to individuals facing end-of-life issues and their families. • A minimum of 2 years of clinical experience as a Licensed Clinical Social Worker.
