• Developing and implementing comprehensive care plans in collaboration with an interdisciplinary team, ensuring personalized approaches that effectively respond to each client's unique circumstances.
• Conducting thorough evaluations of clients to assess their mental health status, family dynamics, and social needs. This may involve in-depth interviews, review of medical histories, and utilization of standardized assessment tools.
• Providing crisis intervention and counseling services, ensuring clients and their families feel supported and empowered throughout their journey.
• Equipping clients and families with relevant resources, referrals, and educational materials aimed at enhancing their understanding and coping strategies regarding mental health and hospice care.
• Utilizing modern practices such as teletherapy and evidence-based therapeutic modalities to improve service delivery and enhance overall client satisfaction.

This role primarily focuses on serving older adults and their families within hospice care, offering a supportive environment as they approach end-of-life issues. Understanding the complexities of chronic illnesses, grief, and loss is paramount, and the LCSW will play a vital role in guiding clients through emotional distress while managing their mental health needs.

• A Master’s degree in Social Work from an accredited institution, providing a strong theoretical foundation in clinical social work principles.
• An active LCSW license issued by the state of Nevada, verifying capability and legal authorization to practice in the state.
• Demonstrated experience in conducting psychosocial assessments and crafting appropriate interventions.
• Strong understanding of the mental health landscape and issues affecting older adult populations, with a particular emphasis on hospice care.
• Exceptional communication skills, both verbal and written, particularly in delivering sensitive information with tact and empathy.

• Two or more years of relevant work experience, particularly in hospice, palliative care, or geriatric mental health, is preferred.
• Experience utilizing electronic health records (EHR) systems for documenting patient interactions and managing care plans.
• A background in multidisciplinary team collaboration, showcasing an ability to work effectively with nurses, physicians, and other healthcare providers.
