Position Overview

Healing Arts Social Work Services is seeking a dedicated and licensed Master's Level Social Worker to join our team in a full-time capacity. This remote position will allow you to engage with seniors and their families from the comfort of your own home while playing a pivotal role in enhancing their quality of life. You will be responsible for providing essential psychosocial support and guidance, helping to address the unique challenges faced by our senior client population.

This role is entirely remote, offering the flexibility to manage your schedule while maintaining consistent communication with clients via video calls, phone conversations, and electronic health record (EHR) platforms. Key Responsibilities

Your role as a Social Worker will be both challenging and rewarding. Key responsibilities include:

  • Conducting comprehensive psychosocial assessments to identify the emotional, social, and mental health needs of seniors.
  • Collaborating closely with clients and families to create and implement personalized care plans that align with their specific goals and preferences.
  • Providing counseling and emotional support during life transitions, such as relocation to assisted living or coping with the loss of a loved one.
  • Advocating for clients to ensure their rights are upheld and that they have access to necessary community resources.
  • Working alongside a team of nurses, caregivers, and healthcare providers to enhance client care and address complex situations.
  • Regularly visiting clients to assess their living conditions and well-being, and modifying care plans as necessary.
  • Facilitating family meetings to improve communication and resolve any conflicts regarding the care of senior clients.
  • Educating clients and their families on community resources, including support groups and wellness programs that promote social engagement.
  • Maintaining accurate documentation of client assessments, treatment plans, and progress to comply with ethical standards and regulations.
  • Staying updated on best practices in social work and relevant policies to ensure the highest quality of care.
Patient Population / Client Focus

This role focuses primarily on senior clients, understanding their unique needs as they navigate aging. You will address various issues, including mental health challenges, social isolation, and access to healthcare. By fostering strong, trusting relationships, your work will significantly impact their overall well-being.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Must hold a valid state licensure as a Licensed Master Social Worker (LMSW).
  • Minimum of two years of experience in social work, preferably within a community or geriatric setting.
  • Comprehensive knowledge of social work principles, aging-related issues, and local community resources.
  • Exceptional skills in assessment, counseling, and problem-solving.
  • Strong ability to empathize and communicate effectively with seniors and their families.
  • Experience with electronic health record (EHR) systems and proficiency in using computer applications for documentation.
Preferred Experience
  • Experience in geriatric social work or community-based services is highly preferred.
