This role will be primarily remote, allowing for flexibility while focusing on our patients' critical needs. As a social worker based at a hospice center, you will engage with patients and their families needing emotional and psychological support during challenging times. • Conduct comprehensive patient assessments through direct interviews, gathering pertinent mental health histories from patients and their families to inform care.
• Apply your specialized knowledge in mental health to formulate diagnoses and develop tailored treatment plans and therapeutic interventions.
• Document patient interactions, treatment progress, and ensure the maintenance of accurate and up-to-date patient records.
• Actively respond to patient inquiries and issues, ensuring that referrals are followed up in a timely manner, fostering open communication throughout the care process.
• Occasionally serve as the on-call clinician, able to address urgent patient needs during office hours, thereby ensuring continuity of care.
• Work collaboratively with interdisciplinary teams, including primary care providers, psychiatrists, and community services, to deliver holistic behavioral healthcare.

You will work primarily with patients in hospice care, which necessitates a compassionate approach to end-of-life issues, facilitating conversations about grief, loss, and emotional well-being. Understanding the delicate nature of this population is crucial to providing effective support and interventions.

• A Master’s degree in Social Work from an accredited program.
• A valid and active Social Worker license (LCSW) to practice independently in Nevada.
• Exceptional skills in communication and relationship management, with a proven ability to connect with diverse clients.
• Strong expertise in psychosocial assessments, clinical interventions, and therapeutic treatment planning.
• The ability to work effectively with teams across various disciplines to achieve comprehensive patient care.

• At least two years of clinical experience in social work, preferably within healthcare or hospice settings.
• Demonstrated capability in addressing complex emotional issues related to life-limiting diagnoses and grief counseling.
• Experience with electronic medical record (EMR) systems to manage and document patient information effectively.
