Key Responsibilities
  • Conduct comprehensive assessments to identify the unique needs of each patient, ensuring personalized treatment plans are developed.
  • Facilitate communication between various stakeholders, including family members, guardians, and other involved parties, regarding patient progress and treatment updates.
  • Create and share a tentative schedule of appointments, home visits, and observations with supervisors to track work progress and outcomes.
  • Coordinate home visits for in-depth observations and to gather relevant data for client care.
  • Ensure effective implementation of treatment plans by collaborating with clients and families, adjusting as necessary based on progress.
  • Participate actively in Interdisciplinary Team Meetings to discuss patient cases and share insights and strategies.
  • Complete detailed monthly and quarterly progress notes to document client advancements and challenges.
  • Organize training for staff members to ensure competency in the implementation of treatment plans and maintaining proper documentation.
  • Conduct follow-up interviews and home visits to provide ongoing support tailored to the evolving needs of clients.
  • Maintain organized records to ensure quick access to pertinent information for all stakeholders.
  • Act as a liaison with the community and other mental health organizations to foster collaborative care approaches.
  • Assist in developing training materials aimed at improving treatment programs for diverse populations.
Patient Population / Client Focus

In this role, you will work with a diverse population, including adults and youth facing a variety of mental health challenges. Your expertise will cater to individuals experiencing issues such as anxiety, depression, trauma, and relational conflict. Qualifications
