We bring multidisciplinary virtual care to patients at community specialty care practices, particularly GI and hepatology brick & mortar clinics around the US.  We assign each patient a "whole patient" care team and offer daily access to app-based care, using Remote Patient Monitoring ("RPM") and Chronic Care Management ("CCM") to bill. These services are covered by most patient's insurance, both Medicare and commercial.
