• Conduct thorough and compassionate assessments of patients’ social, emotional, and psychological needs, devising tailored treatment plans that reflect individual circumstances and goals.
• Implement evidence-based therapeutic interventions, focused on alleviating symptoms of anxiety, depression, and trauma, and fostering resilience among patients.
• Maintain accurate, confidential documentation of patient interactions and progress, adhering to all regulatory standards and organizational policies.
• Participate in multidisciplinary treatment team meetings to discuss patient care, share insights, and align on intervention strategies that promote cohesive treatment efforts.
• Engage in ongoing professional development by staying informed of the latest research and techniques in social work, ensuring optimal patient outcomes.

Working within the Hospice Center, our LCSWs primarily serve patients coping with terminal illnesses and their families. You will provide not only therapy but also emotional support and guidance during challenging times, creating a safe space for patients to express their feelings and fears while navigating their journeys. Understanding the unique needs of this population will be crucial in building trust and providing effective solutions.

• A Master’s degree in Social Work from an accredited institution.
• An active LCSW (Licensed Clinical Social Worker) license in the state of Georgia.
• Demonstrated excellent communication and problem-solving skills, with a patient-centric approach to care.
• A passion for working collaboratively in a team environment, fostering positive relationships with colleagues across disciplines.

While not mandatory, candidates with prior experience in hospice or palliative care settings will be preferred. Familiarity with trauma-informed care practices and solution-focused modalities will enhance your ability to connect with and support our patients effectively.
